Personal Development Plan

What do I want to achieve? 
I want to achieve a D*D*D in Information technology BTEC level 3 extended diploma in 2016. This  is my long term goal. The only way I can achieve this goal is by completing my short term goals which is to get to distinction in all the assignments possible. I also want to achieve 100% attendance to all my lessons by the end of the year.  This is also a vital goal because if my punctuality and attendance is poor I would not be able to achieve my long term goal. 
How important is this goal to me? 
This goal is important to me because it would enable me to get admitted into Kings College London to study computer science and business management. Getting into a Russell group university has always been my dream. Furthermore, achieving this goal allows me to break into the largest industry in the world and it gives me an option of working either in the tertiary sector or the quaternary sector. 
What are the factors that will help me achieve my goal? 
The prominent factor that will help achieve my goal is to work hard and be determined to complete all my assignments before the due date. Another factor that will help me is to be punctual and attend all my lessons. Reviewing my personal development plan frequently will help me achieve my goal as well because It will help me stay motivated and remind me the reason why I am putting in the effort. 
Who can help me achieve my goal? 
My teachers can help me achieve my goals because they will teach me everything that is necessary to get my target grade and my tutor will help me stay on track because she will be monitoring my attendance and punctuality which will help me achieve this goal because she will make sure that I stay above 95%. My friends can also help me achieve this goal because they can support me and encourage me put in the effort. The most important person that can help me achieve this goal is myself because I can have good people around that encourage me to put in the effort and I can still decide not to do the work. So I have to be the person that actually takes the action and motivate myself to put in 110% which I would. 
What are the factors that may stop me achieving this goal? 
Going to parties every weekend may stop me from achieving this goal because I will not get enough rest which in turn, can lead to me turning up for lessons late and missing out on key information. Turning up to lesson without the right equipment will stop me from achieving my goal because I would not be able to note down key information that I might need in order to get the top grade in my assignments.  
What is the gap between where I am now in terms of my skills and knowledge, and where I want to be? 
The skills and knowledge that I need to be where I want is the skill of perseverance because things are going to get hard on this course so I need the skill to keep on going when it is tough and keep myself motivated. I also need to know all the components on a motherboard which I am learning and I need to understand all the abbreviations related to my course. In addition to this I need to understand all the relevant IT terminology which I did not learn in secondary school.
